How to Make a Claim

Personal Accident Claims

It is a requirement of the policy that all claims need to be lodged within 30 days of the date of injury.  When a player is injured and wishes to submit a claim the following process needs to be followed.

• A claim form needs to be obtained from OAMPS Laser Insurance.  This can be obtained either by calling 1800 SPORT 1 (1800 776 781) or downloading from this site.     

• The player or parent/guardian (in the case of a claimant under the age of 18) is to fully complete the claim form and arrange for the treating doctor to complete the “Medical Statement” section of the form.  If claiming loss of income the claimant’s employer is to complete the “Employment Details” section.  When completed the claim form should be returned to the club secretary.      

• The club secretary is required to review the completed claim form and complete and sign the club declaration section of the claim form and forward to: OAMPS Laser Insurance, GPO Box 1113, Brisbane QLD 400. 

• An “Acknowledgement of Claim” advising claim number will be forwarded direct to the claimant within 7 days of receipt by our office.  Any incomplete details or additional requirements will also be advised at this time. 

• If claiming ongoing loss of income it is necessary to provide medical  certificates at least every 2 weeks. 

• Itemised receipts for reimbursement should periodically be sent to our office together with the claim number and claimant name.

Public & Products, Professional and Directors & Officers Liability Claims

It is essential that you notify OAMPS LASER Insurance immediately on 1800 SPORT 1 (1800 776 781) of any potential claim.  OAMPS LASER Insurance will then provide you with advice as how to proceed.
