Claim forms for injuries covered under the personal accident insurance program are obtained from the CAMS National office by phoning 03 9593 7777.  At that time CAMS personnel will ensure that an incident report form has been received relating to the injury (refer to the table below for Incident Reporting).

Once received, the claim form should be completed and returned to OAMPS Insurance Brokers Ltd, PO Box 852, East Melbourne, Victoria  8002.
