HOW TO MAKE A CLAIM                             Download Claim Form

Personal Accident Insurance

To lodge a Claim firstly you must complete a claim form. Claim forms can be obtained by:

2. Request a claim form be posted to you by calling 1800 SPORT 1 (1800 776 781)

Note: To report major incidents (i.e. Death and Permanent Disability or likelihood thereof) after hours please telephone 0418 836 823.

Medical Treatment Claims:

— The Health Insurance Act 1973 prohibits the Insurer from reimbursing any expenses that are
     either partly or fully covered by Medicare including the “Medicare gap”. These expenses include
     but are not limited to Doctors, Surgeons, Anaesthetists, Xray or MRI scans.

— If claiming for treatment from an ancillary provider a referral from a legally qualified medical
     practitioner (i.e.General Practitioner or Specialist) must be obtained prior to the commencement
     of treatment. Note: The cost of obtaining this referral or any costs associated with the completion
     of the “Medical Statement” are not claimable under this policy.

— Original receipts must be forwarded for reimbursement.

— If the claimant is covered by a Private Health Fund then a claim must first be submitted to that Fund.
    The statement of benefit provided by the Private Health Fund should then be submitted with the claim.

Loss of Income Claims:

— The Medical Statement must be completed by your treating legally qualified medical practitioner (i.e.
    General Practitioner or Specialist). If continuing to claim loss of income benefits a new certificate is
    required to be submitted at least every 14 days.
